No concern over the navigation distortion (that's just a bad screenshot), the ad that's floating over the text is weird though.

In source I see:

So it was added but ad still there and not reported in logs.

That's intentional, because until late 4.x/early 5.x you can't limit a the "add to all pages" filter to specific sites.
